A notable incident involved media personality Yaakov Bardugo. According to the report, a page of specific instructions from the PMO was sent to Bardugo, who mistakenly published the document in its entirety. When the error was realized, Bardugo reportedly claimed the notes were his own personal reflections rather than dictated talking points from the Prime Minister's bureau.

The findings also point to what is described as a "cynical move" during a meeting between the Prime Minister and the families of hostages held in Gaza. The correspondences suggest that interactions with the families were managed with a focus on political optics and narrative control rather than purely humanitarian or operational transparency.
