According to ZAKA, Mizrahi was shot in what appears to be a friendly-fire incident by security forces. His son, who lives in Montreal, was unable to reach the hospital for hours; police eventually informed him that his father had been killed earlier at the scene. ZAKA chief Baruch Nidam said the family contacted the organization and is being assisted.

At 07:42 Tuesday, Montreal police announced that an officer had shot Mizrahi after mistaking him for a suspect — the first official admission of friendly fire.
